29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about 10/15/2018 in the machine shop, employee's finger was amputated when his finger got caught between two parts as he cycled the broach machine. b) On or about 11/20/2018 in the Tool Room, when employees were operating the Vertical Milling Center 2 Hurco #1 employees were exposed to caught-by hazards when there is no interlock to prevent the doors from being opened when the machine is in operation. c) On or about 11/20/2018 in the Tool Room, when employees were operating the Vertical Milling Center 2 Hurco #2 employees were exposed to caught-by hazards when there is no interlock to prevent the doors from being opened when the machine is in operation. d) On or about 11/20/2018 in the Tool Room, when employees were operating the Vertical Milling Center Hardinge 1000II employees were exposed to caught-by hazards when there is no interlock to prevent the doors from being opened when the machine is in operation. e) On or about 11/20/2018 by the front shop office, when employees were operating the Vertical Milling Center Hardinge 100 #0 employees were exposed to caught-by hazards when there is no interlock to prevent the doors from being opened when the machine is in operation.
