Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly. a) On or about October 19, 2018, or prior thereto: The Caterpillar powered industrial truck, model# GP25, S/N #SAM91562, was modified with a front attachment to lift wood onion bins in the facility. The onion bins weigh up to 1,750 pounds when full. The modification was made without manufacturer's prior written approval, and instruction plates were not modified after the attachment was added to the powered industrial truck. NOTE: Abatement certification is required for this item.
