29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On or about 10/01/2018, at the jobsite located at 9970 Page Avenue, St Louis, MO, the forklift operators were allowed to operate powered industrial trucks without completing the driving evaluation element of the training requirements as specified by the OSHA Standard.
