Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1926.1053(b)(15): Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. a. On or about July 16, 2018, at a site located at 8675 York Road in Pataskala, Ohio, employees were exposed to fall hazards while using a Werner 24 foot fiberglass extension ladder, Model #D6224-2. The ladder had visible structural defects that included, but were not limited to, split side rails. Employees that were preparing to install translucent roof panels used the ladder to access the roof of a new build, single story, water treatment plant that was approximately 15 feet tall. The employer did not ensure that the ladder was inspected prior to it being used at the site, or on a periodic basis.
