29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) Ammonia Refrigeration System Machine Rooms #1 and #3; On or about June 12, 2018, and at times prior, the employer exposed employees to asphyxiation hazards, in that the ventilation systems were not designed to be automatically activated by an ammonia leak detection system as required by IIAR 2-1999.
