29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 6/5/2018, Silo Area: Employee performing maintenance on saw dust transfer equipment did not lock out rotating parts. b) On or about 6/13/2018, Saw Mill: Employees performing operations such as, but not limited to cleaning, greasing and blade changes were not locking out equipment such as, but not limited to chains, conveyors, debarker, and saws. Abatement certification is required for this item.
