29 CFR 1910.119(f)(1)(iii)(B): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. a) On or about May 7, 2018 or prior thereto, in the Sauce Room at Pod 2, Contherm 5, E-17: Standard Operating Procedure (SOP) IV2 was not fully implemented to prevent employee exposure to an ammonia release measured at 200 ppm at the source of the leak.
