29 CFR 1910.1000(a)(2): Employer did not protect employees exposed to airborne concentration of carbon monoxide (CO) listed on Table Z-1 in excess of the 8 Hour TWA concentration of 50 ppm: On April 08, 2018, an employee was overexposed to carbon monoxide (CO) from the boiler (also known as a "stove") while removing large wood chunks from inside a fuel burner bin (hopper). The employee's 8-hour Time Weighted Average (TWA) exposure was 123.6 ppm, which is 2.47 times the OSHA permissible exposure limit of 50 ppm.
