29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: On or about April 05, 2018 and at times prior thereto, at the Trail Ridge Drive Trenching/Utilities Project, the employer did not ensure employees working in an excavation were protected from loose rock and soil exposing employees to struck-by and engulfment hazards.
