29 CFR 1910.178(a)(6): On each powered industrial truck, the user did not see that all nameplates and markings are in place and are maintained in a legible condition: (a) On the dry dock which was holding M/v Matanuska at Vigor Marine, LLC shipyard at 5555 N Channel Ave, Portland, Oregon: On or about 2/7/2018 and at times prior thereto, employees were exposed to struck by and rollover hazards, in that on forklift #F30, the user did not ensure that all nameplates and markings were in place and maintained in a legible condition, and the control lever knob was missing on the controller which would raise or lower the mast.
