29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): Instances including, but not limited to, the following: a) Job site, 500 Scotti Drive, Bridgeville, PA: On or about October 19, 2017, during set up activities on the Optimat KAL 310 Edgebander unit, the employer did not ensure that all energy isolating devices were located and operated so as to isolate power to the machinery, resulting in an amputation injury occurring to an employee attempting to make an adjustment to the machinery as it was operating.
