29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which was or may be energized. The specific safety-related work practices were not consistent with the nature and extent of the associated electrical hazards. a. Wilbert, Inc. located in Bellevue, Ohio: On or about October 23, 2017, the employer did not ensure a maintenance employee repairing Press A8 shut off the main electrical disconnect and placed a lockout device on the disconnect prior to replacing a heater band on the barrel. b. Wilbert, Inc. located in Bellevue, Ohio: On or about October 23, 2017, the employer did not ensure a maintenance employee repairing Press C2 placed a lockout device on the main electrical disconnect before verifying zero energy on the heater band. c. Wilbert, Inc. located in Bellevue, Ohio: On or about October 23, 2017, the employer did not ensure a maintenance employee repairing Press C2 and A8 performed a functional check of his voltage meter before using the meter to test for the presence or absence of electrical energy on the heater bands.
