29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment: The employer is failing to protect employees from explosion and fire hazards by failing to establish and implement written procedures to maintain process equipment in the lead styphnate manufacturing process. This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri, regrading the following equipment: a) Written procedures to ensure mechanical integrity of equipment were not established or implemented for the lead nitrate valve in the lead styphnate manufacturing process. b) Written procedures to ensure mechanical integrity of equipment were not established or implemented for the magnesium styphnate valve in the lead styphnate manufacturing process. c) Written procedures to ensure mechanical integrity of equipment were not established or implemented for the Delta V Control System Monitor and associated transmitting equipment, monitoring equipment for the lead styphnate manufacturing process.
