29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. On or about 7/31/2017 an employee was exposed to a fall of approximately 17 feet. The employee was performing painting activities on a residential home located at 7679 Bear Swamp Road, Wadsworth, Ohio 44281. The employee was working from a 30 foot extension ladder that was not set with the angle one quarter the working length of the ladder.
