29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). 918 N. Porter St. in Streator, IL 61364, warehouse, powered industrial trucks (counter-balanced propane-gas powered forklifts): on or about July 17, 2017, employees were permitted to operate forklifts for regular mechanical material handling tasks without receiving adequate operator's initial training in truck-related and workplace-related topics through formal instruction, practical training, and evaluation applicable to assigned work tasks to ensure competency in safe operation. Employees were exposed to struck-by hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
