29 CFR 1910.146(k)(1)(ii): The employer did not evaluate a prospective rescue service's ability, in terms of proficiency with rescuerelated tasks and equipment, to function appropriately while rescuing entrants from the particular permit space or types of permit spaces identified. a) Warehouse, 1500 Corporate Blvd, Newburgh, NY - On or about 4/15/2017, an employee performing troubleshooting and maintenance activities on a bot charging station in a permit required confined space, sustained serious hand injuries when his hand got caught in between the structure and the MVC (multi-level vehicle conveyor). The rescue services identified on the permit was a 100% volunteer department and did not have a Confined Space rescue team.
