29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about April 25, 2017, at the trenching site located at 1728 E. 14th Place in Tulsa OK, no protective devices were in place to protect an employee working in a trench that was more than 5 feet in depth. The employee was exposed to struck-by and crushed-by hazards.
