29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about April 25, 2017, at the steel distribution and fabrication facility located at 2400 N.E. 36th Street, Fort Worth, Texas the employer used a Hyundai Model 250D-7E, Diesel fork lift attached with a Brute Lift Boom without written approval from the manufacturer to use the boom as an attachment exposing employees to crush-by hazards.
