29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry. a) On or about February 17, 2017 and at times prior thereto, at the Unit 1 Preheater Alpha Crossover Duct, the employer did not develop procedures for summoning rescue and emergency services for employees working and replacing an expansion joint. b) On or about February 17, 2017 at the Unit 1 Preheater Alpha Crossover Duct, the employer did not prevent unauthorized employees from entering.
