29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: On January 17, 2017 at a sewer manhole located approximately six feet southwest of building "G", the sewer manhole was not evaluated to determine if the manhole was a permit-required confined space. Employees were exposed to engulfment, toxic gas, explosive/flammable gas, and/or oxygen deficiency hazards.
