29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Thermoplastics, Hyster S50XM powered industrial truck: on or about November 8, 2016, modifications that affected the truck's capacity and safe operation were made to fork attachments without the manufacturer's prior written approval. Holes were drilled in the forks for hoisting, lifting, and transporting suspended loads (molds). Employees operating equipment, performing setup, and working around the moving truck with suspended loads were exposed to struck-by and/or crushed-by hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
