29 CFR 1910.1048(l)(1)(i): The employer shall institute medical surveillance programs for all employees exposed to formaldehyde at concentrations at or exceeding the action level or exceeding the STEL. a) On or about October 4, 2016 the employer had not implemented a medical surveillance program for employees working on the liquid lines in the Production Department that had previous air monitoring results above the Action Level of 0.5 PPM for Formaldehyde.
