29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects: a) 1819 Ginnoda St, Philadelphia, PA - Employees working from a fabricated frame scaffold approximately 30 feet above the ground did not install toeboards on the scaffold thus exposing the employees working below to struck-by hazards. This condition was observed on or about September 16, 2016.
