29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On August 24, 2016 the employer did not ensure that energy control procedures were utilized to control the electrical, gravitational and pneumatic energy sources when employees were performing die changes on the mechanical power press. Employees were exposed to crushing hazards while performing this operation.
