29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to protect maintenance employees who service and maintain equipment from hazards associated with the unexpected energization / startup / release of stored energy. This was most recently documented on July 26, 2016 at the establishment located at 101 Buchanan Street and 44 Dock Street, St. Louis, MO. The employer failed to develop and document procedures for the control of potentially hazardous energy for the following equipment with multiple energy sources: a. 101 Production Line, consisting of the Fischbein-Inglett bagging machine, Pasco Palletizer, Premier Tech Bag Scale (electric, pneumatic, mechanical, gravity) b. Dock Street Production Line, consisting of a Premier Tech Chronos Bagging machine PTH-920, Premier Tech Robotic palletizer AR-215, Premier Tech Bag Scale (electric, pneumatic, mechanical, gravity) c. Bratney Jug Production Line (electric, pneumatic, mechanical, gravity) d. Multiple belt and bucket conveyor systems (electric, mechanical, gravity) Such typical hazardous energy sources include, but may not be limited to, mechanical (e.g., power transmission apparatus, counterbalances, springs, pressure, and gravity), pneumatic, hydraulic, electrical, chemical, and thermal (high or low temperature) energies that could cause injury to employees.
