29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the team's findings and recommendations. Instance a) This violation was most recently observed on or about June 21, 2016 and times prior thereto and times thereafter, in the Refrigeration Process at American Cold Storage (ACS) located in Tulsa, Oklahoma when the employer did not address the 2009 PHA team's findings and recommendations including but not limited to implementing corrosion inspections when insulation was removed, missing and/or damaged on piping system. Instance b) This violation was most recently observed on or about June 21, 2016 and times prior thereto and times thereafter, in the Refrigeration Process at American Cold Storage (ACS) located in Tulsa, Oklahoma when the employer did not address the 2009 PHA team's findings and recommendations including but not limited to conducting emergency mock drills.
