29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) On or about June 16, 2016 -- Building 1, Mezzanine at 7400A; End of mezzanine platform adjacent to the fixed ladder access was not guarded with a standard guardrail on the open side facing the bus when short buses were in the bay. b) On or about June 16, 2016 Building 1, Mezzanine at 7400A; Fall protection was not in place along the edges of the mezzanine in front of the bus window openings to prevent employees from slipping and falling through the openings into the bus body below. c) On or about June 16, 2016 -- Building 1, Mezzanine at 4200A, End of mezzanine platform adjacent to the crosswalk; No guard rail was in place to protect employees working beside an unguarded bus window opening. d) On or about June 16, 2016 -- Building 1, Bays 3400 and 3600; Guarding was not provided for employees installing air tanks and other equipment inside of bus shells. Employees using wheel wells as work platforms were exposed to an unguarded 4-foot by 4-foot bus opening that was 59 inches above the floor level. e) On or about June 9 and June 23, 2016 -- Building 6, Welding Carts; At least two welding carts used as work platforms were not provided with standard guardrail on all open sides. Side rails on carts did not extend to the edge of the platforms' sides, leaving a space where employees could fall off the end of the carts or between the railing and the work jigs. f) On or about August 24, 2016 -- Building 6, Welding Carts; A welding cart used as work platform was not provided with standard guardrails on all open sides. Side rails on the cart did not extend to the edge of the platform's sides, leaving a space where an employee could fall off the end of the cart or between the railing and the work jig.
