29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about May 13, 2016, and at times prior thereto, employees were exposed to an ejection/crushing/struck-by hazard, while operating a Yale forklift (Model GP050TGE85) to transport materials without being trained in its safe use and operation.
