29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railings (or equivalent) on all open sides except where there was an entrance to a ramp, stairway, or fixed ladder. a) On or about May 12, 2016, in the Hydro Department, Building 155, between columns G:12 and G:13, employees accessing the electrical platform by means of a fixed ladder were approximately 14 feet above the floor was missing the midrail exposing employees to fall hazards. b) On or about June 1, 2016, employees entering and exiting Building 200 at the loading dock there were no guardrails installed exposing employees to falls greater than 4 feet. c) On or about June 1, 2016, employees entering and exiting Building 195, the loading dock was approximately 5 feet 6 inches above ground level had no guardrails exposing employees to fall hazards.
