29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use: a) Maintenance Shop, on or about 4/29/2016: Two karts containing oxygen and acetylene tanks were in storage but kept connected with their regulators. Employees are exposed to a fire hazard.
