29 CFR 1910.119(e)(3)(vii): The process hazard analysis did not address a qualitative evaluation of a range of the possible safety and health effects of failure of controls on employees in the workplace. In the facility at 106 Lonczak Drive Chicopee, MA: On or about April 13, 2016, the Process Hazard Analysis (PHA) revalidation dated April 13, 2013, for the covered heat treating process did not include a qualitative evaluation of a range of possible health and safety effects of failure of controls in the workplace for each of the "What-If" scenarios included in the PHA in that none of the scenarios listed health and safety effects such as, but not limited to, burns and smoke inhalation.
