29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: a) Facility, interior wall, front of building: The outlet located inside the store near the foyer was wired so the hot and neutral were reversed, on or about 2/24/2016. b) Facility, foyer: The outlet located inside the foyer on the wall shared by the manager's office was wired so the hot and neutral were reversed, on or about 2/24/2016. c) Facility, Outside the Manager's Office: The outlet located outside the manager's office was wired so the hot and neutral were reversed, on or about 2/24/2016 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19
