29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart. (a) 1857 Pebble Court, Harrisburg Pennsylvania - The competent person did not conduct an adequate visual and manual test to determine the soil classification and appropriate method of protection. An employee became trapped when he entered the trench, on or about September 30, 2015. (b) 1857 Pebble Court, Harrisburg Pennsylvania - The competent person did not follow the requirements set forth in Appendix B, to correctly determine the required sloping above the box and position of the trench box in type B soil, on or about October 5, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
