29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: (a) On or about October 26, 2011, and times prior, Zavanna LLC, as controlling employer, did not ensure that Precision Drilling Company LP, a subcontractor, protected their employees from injury in that, the fire extinguisher located at the mixing area was not mounted and was in an area where materials were stacked preventing safe access to it.
