29 CFR 1910.269(m)(3)(ii): All switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized shall be open. Such means inoperable, unless its design does not so permit, and such means shall be tagged to indicate that employees are at work. (a) Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757: On or before July 10, 2015, all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized were not open. The disconnect switches for Feeder 1 were located about a mile away and were not opened to deenergize the back feed to the South Woodrow Substation. On July 10, 2015 an employee was electrocuted when he came into contact with 7.2 kV, phase to ground. This condition exposed employees to electric shock and/or an electrocution hazard.
