1910.119(e)(5): The employer did not establish a system to promptly address the team's findings and recommendations, did not ensure the recommendations were resolved in a timely manner and that the resolutions were documented; in addition documentation of what actions were to be taken, development of a written schedule of when these actions were to be completed, and communication of the actions to operating, maintenance and other employees whose work assignments in the process areas may be affected by the recommendations or actions was not completed: (a) Throughout the facility; The employer did not establish a system to promptly address the 2011 PHA findings and recommendations as none of the items had been noted as completed on 6/15/2015; action item dates were scheduled out as far as the end of 2015.
