29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about February 9, 2015 and in times prior thereto, a forklift operator was driving without wearing his seat belt exposing to employee to the hazard of being ejected from the forklift. The forklift was being operated in the outside utility area and the road around this area.
