Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential amputation hazard or other injury: (a) For employees operating the freight elevator and exposed to the potential for injury created by the lack of doors or gates on the elevator car. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the requirements set forth in ANSI/ASME A17.1-1984 Rule 204.4a: Where Required. A door or gate shall be provided at each entrance to the car. This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in Inspection Number 102763000, Citation Number 1, Item Number 4A, issued on 04/12/2002.
