American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, electrical protective rubber insulating gloves were not inspected every 6 months. Sets of gloves were identified that were last tested in 2009 and 2011. b.) American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, electrical protective rubber insulating blankets were not inspected every 12 months. Blankets were identified that were last tested in 2008.
