29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: This violation occurred on January 21, 2015, and at times prior thereto, in the maintenance area, employees were operating an Amrox bench grinder model # 7550A, s/n not legible without a work rest exposing employees to caught in or struck by hazards.
