29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. A. The company had not instituted adequate housekeeping measures to ensure that combustible wood dust did not accumulate beyond generally accepted guidelines in the following locations: 1. Main Room, Front of Shop: On, or about, November 25, 2014, combustible wood dust had accumulated on surfaces such as, but not limited to, window sills, conduit, a crane structure, heater units, piping and ledges. 2. Back Shop: On, or about, December 1, 2014, combustible wood dust had accumulated on surfaces such as, but not limited to, light fixtures, a wooden pipe enclosure, conduit and piping. B. Side Room, Front of Shop: On, or about, November 25, 2014, water leaked from the roof onto the Minimax T 401 shaper. The machine had to keep covered with a tarp when not in use.
