29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person. Dusit Thani Hotel Jobsite, Sixth Floor, Banquet Room 1 and 2: Pacific Rim Land Development permitted a four tier fabricated frame orange mobile scaffold to be moved by employees not under the supervision and direction of a competent person qualified in moving mobile scaffolds. On October 18, 2014, the mobile scaffold toppled over fatally injuring a plumber. The scaffold was hazardous in that the scaffold frame legs were not designed with holes to accept mobile scaffold wheel stem pins to prevent the wheels from coming out of the frame legs; and the height to base ratio of the mobile scaffold was greater than 4 to 1. When the orange scaffold toppled it was measured to be 16 feet, 7.5 inches tall and 4 feet wide.
