29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): On or about October 1, 2014, employees were exposed to fall hazards of approximately 14 feet when the opening used to place pallets of materials on the mezzanine was not protected by closing the gates. A single chain with streamers had been placed across the opening.
