The Andersons, Inc. located in Maumee, Ohio: On or about September 23, 2014, the employer did not ensure inspection and maintenance procedures were established and implemented to maintain the on-going integrity of the Corken 521 EG pump which supplies anhydrous ammonia to the process. b. The Andersons, Inc. located in Maumee, Ohio: On or about September 23, 2014, the employer did not ensure inspection and maintenance procedures were established and implemented to maintain the on-going integrity of the Fisher and Squibb Taylor globe valves located on anhydrous ammonia process lines. c.
