29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. (a) Front Office - On or about September 18, 2014 employees were exposed to electric shock hazards when accessing a electrical circuit breaker box with live parts. Employees were not protected from the missing breakers by covers over the live parts. (b) Northeast Corner of Shop - On or about September 18, 2014 employees were exposed to electric shock hazards while connecting or disconnecting flexible cords to outlets. The outlets were missing covers and knock-outs.
