29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required: (a) Georgetown Wastewater Treatment Lagoon: On August 28, 2014, two employee(s) entered a confined space (5' x 5' x 18.5') without receiving adequate instruction as to the hazard(s) of entering this confined space to perform the spray application of Flex Seal sealant. Flex Seal contained flammable chemicals including toluene, parachlorobenzotrifluoride, acetone, heptane, isobutane, and propane. The Flex Seal was sprayed inside of a confined space that was approximately 387 cubic feet in air volume plus 75 cubic feet of water volume. Equipment including, but not limited to ventilating fans, respiratory protection, harnesses, lifelines, and/or emergency rescue equipment were not used to protect employees. NOTE: Abatement certification and documentation are required for this item.
