29 CFR 1910.333(b)(2)(iv)(A): Before circuits or equipment were worked as de-energized, a qualified person did not operate the equipment operating controls or otherwise verify that the equipment could not be restarted: On or about June 2, 2014, the employer failed to ensure employees performing high voltage regulator tests were protected against accidental contact with approximately 6900VAC by verifying no voltage was present.
