29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts are deenergized: The employer does not ensure before circuits or equipment is worked as de-energized, a qualified person use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and verify that the circuit elements and equipment parts are de-energized. a. This violation occurred on or about August 4, 2014, in the Lung Pump Room where employees were exposed to approximately 290 VAC electrical hazards when replacing Scalder Blower motor #5 without having a qualified person using test equipment to test circuit elements and electrical parts of equipment to which employees would be exposed and verify that the circuit elements and equipment parts are de-energized. b. This violation occurred on or about August 4, 2014, in the Lung Pump Room where employees were exposed to approximately 290 VAC electrical hazards when replacing Scalder Blower motor #6 without having a qualified person using test equipment to test circuit elements and electrical parts of equipment to which employees would be exposed and verify that the circuit elements and equipment parts are de-energized. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that before circuits or equipment is worked as de-energized, a qualified person use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and verify that the circuit elements and equipment parts are de-energized.
