29 CFR 1910.335(a)(2)(ii): When normally enclosed live parts were exposed for maintenance or repair, they were not guarded to protect unqualified persons from contact with the live parts: a. On or about June 25, 2014, an unqualified employee was repairing an energized florescent lamp ballast and not guarded to protect this employee from contact with live parts which resulted in the employee sustaining an electrocution-type fatality.
