29 CFR 1915.89(b): The employer did not establish and implement a written program and procedures for lockout and tags-plus systems to control hazardous energy during the servicing of any machinery, equipment, or system in shipyard employment. F/V Alpine Cove, St Hermon Harbor, Kodiak, AK: On or about 6 June 2014 and times prior thereto, the employer did not have a lockout/tags-plus procedure in place. Employees were performing hot work in the engine room on a R-22 Freon refrigeration system which was not locked or tagged out of service. This condition exposed employees to chemical and asphyxiation hazards from a R-22 Freon leak on the vessel.
